My Macbook is slowing down

i own 2009 macbook, 2.26 4 gb of ram. disc drive 3/4 full.
have been having many more 'beach ball moments' lately. did have reboot off install disc once , restore off external backup hard drive.
repair permissions has been done multiple times, , have done repair disc has well.
downloaded smart utility , ran it, came these results:

hd having issues. and, when using external hd has startup, seems running faster.

so, question is, reformatting internal drive fix these errors, or solution replace it? have applecare next 8 months, install larger hd if have replace anyway.
in advance help.

Permissions Problem?

hello. unable log on computer. took mac store , said had problem permissions. reset computer's permissions , able log onto computer. however, since dock resets day had permissions problem. example, had various cs5 programs on dock. when upgraded cs5.5, replaced cs5 shortcuts cs5.5 counterparts. however, when restarted computer, cs5 icons still there. also, whenever restart computer, browser history resets whatever on day encountered permissions problem. can not save bookmarks. also, files required type in password whenever try moving or deleting file. there other problems wanted see if knows problem have before adding more problems. thank   Forums Macs macOS macOS iPhone Mac OS & System Software iPad Apple Watch Notebooks iTunes Apple ID iCloud Desktop Comput...

iPhone won't sync with iTunes
